How to obtain access to a family member's chart

Obtaining access to a family member's medical record through MyChart allows you to see all medical information and history, including test results, care team messages, appointments, and more. You can only access a family member’s medical record if you are a spouse, parent, guardian, or the legal patient representative.

To request access to a family member’s medical record, log in to your own MyChart account and complete the online record authorization form. You can find the form under the Access Settings submenu.

Please note: Patient representatives must be at least 18 years old. Marshall Medical offers parents/guardians medium proxy access. In accordance with California state confidentiality laws specific to teens, proxy access converts to a medium proxy access on the child's 12th birthday and proxy access terminates on the child's 18th birthday.

Birth to 11 years old: Parents and legal guardians are provided with full proxy access to their child's MyChart account. Fifteen days before your child's 12th birthday, you will receive a notification alerting you that your access will be transitioned to medium proxy access the day before your child's birthday.

12 to 17 years old: Parents and legal guardians are provided with medium proxy access to their child's MyChart account. Fifteen days before the child's 18th birthday, you will receive a notification alerting you that access will be terminated the day before your child's birthday.
